*Your CORE Subscription Plan includes the cost of certain medications when you choose to fill your prescriptions with a third-party pharmacy vendor(s) connected through the Found Platform. GLP-1 medications and Contrave® are not included in the CORE Subscription Plan fee but Found will work with your insurance carrier to determine your policy’s coverage for these medications but coverage details, pricing, and terms vary by your plan. You may have to pay copays, coinsurance, or a deductible. All medications fulfilled outside of the Found platform and all laboratory service related expenses are not included in your CORE Subscription Plan fee. Prescriptions are at the discretion of your medical provider.
Important: Core Subscription Plans are not available to people who have government insurance plans like Medicare, Medicaid, TriCare, MediCal, or similar programs.

CORE Subscription Offer Terms and Cancellation Policy
Your CORE Subscription Plan subscription fees will not increase during the initial program commitment period (“Original Commitment Term”). After your Original Commitment Term ends, your CORE Subscription Plan subscription will automatically renew for the interval of time detailed under the specific plan time (each a “Renewal Term”) at the then current list price for the Services. If you do not wish to continue your subscription for any Renewal Term, you must cancel your subscription before the end of your then-current term. You authorize Found to charge your default payment method any Renewal Fees (plus applicable taxes) at the start of every Renewal Term, until your CORE Subscription Plan subscription is canceled. Your continued use of the Services after the Renewal Term commences constitutes your agreement to pay the modified Renewal Fee.
You can cancel your subscription at any time by contacting our customer support team at [email protected]. Your cancellation will be effective within 24 hours. You can also cancel in the app by clicking the "Cancel Membership" link in your Account Details page on the app or the portal.
All capitalized terms not otherwise defined in these Offer Terms and Cancellation Policy (“Offer Terms”) will have the meanings set forth in the Terms. In case of a conflict between the provisions of these Offer Terms and the Terms, these Offer Terms will control. The CORE Subscription Plan is only available to members who choose to self-pay.
Monthly CORE Subscription Plan
By choosing the Monthly CORE Subscription Plan, you are agreeing to pay $129 (plus applicable taxes) every 30 days starting on the first billing date. Your Monthly CORE Subscription Plan will automatically renew until you choose to cancel. For each period of 30 days you are enrolled in the program, you will be charged $129, and these subscription fees are final and non-refundable to the extent permitted by law, or unless otherwise expressly stated.
Quarterly CORE Subscription Plan
By choosing the Quarterly CORE Subscription Plan, you are agreeing to make 2 equal payments of $348 (plus applicable taxes) each for a 6-month program subscription for a total of $696 (“Original Commitment Term”). The initial payment will be due at the time of your initial purchase, and the second payment will be due 90 days thereafter.
After the Initial Term, your Quarterly Subscription Plan will automatically convert to a Monthly CORE Subscription Plan at the then current list price for the Monthly CORE Subscription Plan and you will be charged the subscription fee every 30 days beginning on the first billing date until you cancel your subscription.
Depending on when you cancel, you may be eligible for a partial refund of fees paid during the Original Commitment Term, however, for each period of 30 days you are enrolled in the program, you will be billed at the non-discounted price of $129, as set forth in the table below:
6-Month CORE Subscription Plan
By choosing the 6-Month CORE Subscription Plan option, you are agreeing to pay $594 for a 6-month subscription plan (“Original Commitment Term”). The initial payment will be due at the time of your initial purchase.
After the Initial Term, your 6-Month CORE Subscription Plan will automatically convert to a Monthly CORE Subscription Plan at the then current monthly list price for the Monthly CORE Subscription Plan and you will be charged the subscription fee every 30 days beginning on the first billing date until you cancel your subscription.
Depending on when you cancel you may be eligible for a partial refund of Fees paid during the Original Commitment Term, however, for each period of 30 days you are enrolled in the program, you will be billed at the non-discounted price of $129, as set forth in the table below:
What’s included in my PLUS Subscription Plan?

*Self-Pay Subscriptions are not available to members who have health insurance coverage through an in-network health insurance plan. If your insurance plan covers Found medical provider visits, you may be eligible for the PLUS Subscription Plan, Insurance Option and you will be responsible for the PLUS Subscription Plan cost in addition to applicable co-pays, co-insurance, and deductibles as outlined by your insurance carrier. If your insurance plan is not in-network with Found, or your insurance plan does not cover Found’s medical visits, and you choose a Self-Pay Subscription, consultations with medical providers are included in your Self-Pay Subscription Plan. Laboratory service related expenses are not included in your Subscription Plan.
**Your health insurance plan may cover medication costs. Found will work with your health insurance plan to determine your coverage availability, and you will be responsible for applicable copays and other out-of-pocket costs. If you elect a Self-Pay Subscription Plan, your subscription includes the cost of certain non-GLP-1 medications only if you choose to fill your prescription with a third-party pharmacy connected through the Found Platform. GLP-1. GLP-1 medications and Contrave® are not included in any subscription plan fee but Found will work with your insurance carrier to determine your policy’s coverage for these medications. PLUS Subscription Plan Offer Terms and Cancellation Policy for Self-Pay Members
To the extent permitted by law, subscription fees for your PLUS Subscription Plan for self-pay members are final and non-refundable unless expressly stated otherwise. Your PLUS Subscription Plan subscription fees will not increase during the initial program commitment period (“Original Commitment Term”). After your Original Commitment Term ends, your PLUS Subscription Plan subscription will automatically renew for subsequent periods of the same length as your Original Commitment Term (each a “Renewal Term”) at the then-current subscription price (“Renewal Fee”).

Pay Monthly PLUS Subscription Plan, Self-Pay Option
By choosing the Pay Monthly PLUS Subscription Plan, you are agreeing to pay $199 every 30 days starting on the first billing date.Your Pay Monthly PLUS Subscription Plan will automatically renew until you choose to cancel. For each period of 30 days you are enrolled in the program, you will be charged $199, and these subscription fees are final and non-refundable to the extent permitted by law, or unless otherwise expressly stated.
PLUS Subscription Offer Terms and Cancellation Policy for Members Who Pay for Medical Care with Insurance
If you choose to pay for your medical care with insurance, Found’s PLUS Subscription Offer is not a covered benefit under most health insurance plans. This subscription plan does not include the cost of any medications or lab work. Some medications you’re prescribed might be covered by your insurance, but coverage details, pricing, and terms vary by your plan. You may have to pay copays, coinsurance, or deductibles for your medical visits, lab work and prescriptions. Prescriptions are at the discretion of your medical provider.
You can cancel your subscription at any time by contacting our customer support team at [email protected]. Your cancellation will be effective within 24 hours. You can also cancel in the app by clicking the "Cancel Membership" link in your Account Details page on the app or the portal.

Found offers members two ways to pay for their PLUS subscription when their medical care is covered by insurance:
1. Pay Monthly PLUS Subscription Plan, Insurance Option
By choosing the Pay Monthly PLUS Subscription Plan and using insurance to cover your medical visits, you are agreeing to pay $39 every month starting on the first billing date. Your $39 (plus applicable taxes) recurring charges will be charged every 30 days beginning on the first billing date until you cancel. To the extent permitted by law, the monthly subscription fees are final and non-refundable unless expressly stated otherwise.
2. Pay Annually PLUS Subscription Plan, Insurance Option
By choosing the Pay Annually PLUS Subscription Plan and using insurance to cover your medical visits and medicines, you are agreeing to pay $199 annually (which amounts to approx $17/month) starting on the first billing date for the first year of your PLUS Subscription Plan. Your Pay Annually PLUS Subscription Plan will automatically renew annually until you choose to cancel. Your annual recurring charges will be charged every 365 days beginning on the first billing date until you cancel. To the extent permitted by law, the $199 annual PLUS Subscription Plan payment is final and non-refundable.
