What’s included in my CORE Subscription Plan?

• Customized meals plans and workouts, as well as daily lifestyle guidance in our mobile app
• Community that helps you easily connect with other members and get support

*Your CORE Subscription Plan includes the cost of non-GLP-1 medication when you choose to fill your prescriptions with a third-party pharmacy vendor(s) connected through the Found Platform. If you choose to fulfill your prescriptions via a different pharmacy of your choosing, non-GLP-1 medications are not included in your CORE Subscription Plan fee. GLP-1 medications are not included in the CORE Subscription Plan fee but Found will work with your insurance carrier to determine your policy’s coverage for these medications. Laboratory service related expenses are not included in your CORE Subscription Plan.

CORE Subscription Offer Terms and Cancellation Policy

To the extent permitted by law, subscription fees for your CORE Subscription Plan are final, non-refundable and subject to the early termination fee unless expressly stated otherwise. Your CORE Subscription Plan subscription fees will not increase during the initial program commitment period (“Original Commitment Term”). After your Original Commitment Term ends, your CORE Subscription Plan subscription will automatically renew for the interval of time detailed under the specific plan time (each a “Renewal Term”) at the then current list price for the Services.  If you do not wish to continue your subscription for any Renewal Term, you must cancel your subscription before the end of your then-current term. You authorize Found to charge your default payment method any Renewal Fees (plus applicable taxes) at the start of every Renewal Term, until your CORE Subscription Plan subscription is canceled. You acknowledge that if you cancel your CORE Subscription Plan subscription prior to the start of the 6th month of a Renewal Term you will be charged the then-current early termination fee. Your continued use of the Services after the Renewal Term commences constitutes your agreement to pay the modified Renewal Fee and the then-current early termination fee in case of early termination. You can cancel your subscription at any time by contacting our customer support team at [email protected] and your cancellation will be effective in 36 hours. All capitalized terms not otherwise defined in these Offer Terms and Cancellation Policy (“Offer Terms”) will have the meanings set forth in the Terms. In case of a conflict between the provisions of these Offer Terms and the Terms, these Offer Terms will control. The CORE Subscription Plan is only available to members who choose to self-pay.

Pay Monthly CORE Subscription Plan

By choosing the Pay Monthly CORE Subscription Plan, you are agreeing to pay $129 every month starting on the first billing date for a minimum commitment period of 6 months. Your Pay Monthly CORE Subscription Plan will automatically renew each month until you choose to cancel. Your monthly recurring charges will be charged every 30 days beginning on the first billing date until you cancel.  

If you choose to cancel your Pay Monthly CORE Subscription Plan prior to day 151 of the commitment term, you will be charged $129 for any month or partial month in which you were enrolled in the program plus an additional $129 early termination fee, as set forth in this table:

Pay Quarterly CORE Subscription Plan

By choosing the Pay Quarterly CORE Subscription Plan, you are agreeing to make 2 equal payments of $348 (for a total of $696) each for a 6-month program subscription. The initial payment will be due at the time of your initial purchase, and the second payment will be due 90 days thereafter. After the Initial Term, your Pay Quarterly Subscription Plan will automatically convert to a Pay Monthly CORE Subscription Plan at the then current monthly list price for the Pay Monthly CORE Subscription Plan  and your monthly recurring charges will be charged every 30 days beginning on the first billing date until you cancel your subscription.

If you chose to cancel your Pay Quarterly CORE Subscription Plan prior to day 151 of the Original Commitment Term, you will be charged a $129 early termination fee. Depending on when you cancel you may be eligible for a partial refund for the months remaining in your 6-month commitment term, however, in addition to the early termination fee, any month or partial month in which you were enrolled in the program will be billed at the non-discounted price of $129 as set forth in this table:

Pay Biannually CORE Subscription Plan

By choosing the Pay Biannually CORE Subscription Plan option, you are agreeing to prepay $594 for a 6-month program subscription. After the Initial Term, your Pay In Full Subscription Plan will automatically convert to a Pay Monthly CORE Subscription Plan at the then current monthly list price for the Pay Monthly CORE Subscription Plan  and your monthly recurring charges will be charged every 30 days beginning on the first billing date until you cancel your subscription.

If you choose to cancel your Pay Biannually CORE Subscription Plan prior to day 151 of the Original Commitment Term, you will be charged a $129 early termination fee. What’s included in my PLUS Subscription Plan?

• Customized meals plans and workouts, as well as daily lifestyle guidance in our mobile app
• Community that helps you easily connect with other members and get support

*Self-Pay Subscriptions are not available to members who have health insurance coverage through an in-network health insurance plan. If your insurance plan covers Found medical provider visits, you may be eligible for the PLUS Subscription Plan, Insurance Option and you will be responsible for the PLUS Subscription Plan cost in addition to applicable co-pays, co-insurance, and deductibles as outlined by your insurance carrier. If your insurance plan is not in-network with Found, or your insurance plan does not cover Found’s medical visits, and you choose a Self-Pay Subscription, consultations with medical providers are included in your Self-Pay Subscription Plan. Laboratory service related expenses are not included in your Subscription Plan.

**Your health insurance plan may cover medication costs. Found will work with your health insurance plan to determine your coverage availability, and you will be responsible for applicable copays and other out-of-pocket costs. If you elect a Self-Pay Subscription Plan, your membership includes the cost of non-GLP-1 medications only if you choose to fill your prescription with a third-party pharmacy connected through the Found Platform. GLP-1 medication costs are not included in any membership fee.

PLUS Subscription Plan Offer Terms and Cancellation Policy for Self-Pay Members

To the extent permitted by law, subscription fees for your PLUS Subscription Plan for self-pay members are final, non-refundable and subject to the early termination fee unless expressly stated otherwise. Your PLUS Subscription Plan subscription fees will not increase during the initial program commitment period (“Original Commitment Term”). After your Original Commitment Term ends, your PLUS Subscription Plan subscription will automatically renew for subsequent periods of the same length as your Original Commitment Term (each a “Renewal Term”) at the then-current subscription price (“Renewal Fee”). If you do not wish to continue your subscription for any Renewal Term, you must cancel your subscription at least 36 hours before the end of your then-current term. Pay Monthly PLUS Subscription Plan, Self-Pay Option

By choosing the Pay Monthly PLUS Subscription Plan, you are agreeing to pay $199 every month starting on the first billing date for a minimum commitment period of 6 months. Your Pay Monthly PLUS Subscription Plan will automatically renew each month until you choose to cancel. Your monthly recurring charges will be charged every 30 days beginning on the first billing date until you cancel.  

If you choose to cancel your Pay Monthly PLUS Subscription Plan prior to day 151 of the commitment term, you will be charged $199 for any month or partial month in which you were enrolled in the program plus an additional $129 early termination fee.

PLUS Subscription Offer Terms and Cancellation Policy for Members Who Pay with Insurance

If you choose to pay for your PLUS Subscription with insurance, the PLUS Subscription Offer is not a covered benefit under most health insurance plans. To the extent permitted by law, subscription fees for your PLUS Subscription Plan are final and non-refundable unless expressly stated otherwise. Your PLUS Subscription Plan subscription fees will not increase during the initial program commitment period (“Original Commitment Term”). After your Original Commitment Term ends, your PLUS Subscription Plan subscription will automatically renew for subsequent periods of the same length as your Original Commitment Term (each a “Renewal Term”) at the then-current subscription price (“Renewal Fee”). If you do not wish to continue your subscription for any Renewal Term, you must cancel your subscription at least 36 hours before the end of your then-current term. You authorize Found to charge your default payment method any Renewal Fees (plus applicable taxes) at the start of every Renewal Term, until your PLUS Subscription Plan subscription is canceled. Your continued use of the Services after the Renewal Term commences constitutes your agreement to pay the modified Renewal Fee. You can cancel your subscription at any time by contacting our customer support team at [email protected] and your cancellation will be effective in 36 hours. All capitalized terms not otherwise defined in these Offer Terms and Cancellation Policy (“Offer Terms”) will have the meanings set forth in the Terms. In case of a conflict between the provisions of these Offer Terms and the Terms, these Offer Terms will control.

Payment of the PLUS Subscription Plan is not a prerequisite for receiving medical care by a Found-affiliated medical provider. Please contact [email protected] for details on how to access medical care without paying the PLUS Subscription Plan fee.

Pay Monthly PLUS Subscription Plan, Insurance Option

By choosing the Pay Monthly PLUS Subscription Plan and using insurance to cover your medical visits and medicines, you are agreeing to pay $39 every month starting on the first billing date. Your Pay Monthly PLUS Subscription Plan will automatically renew each month until you choose to cancel. Your recurring charges will be charged every 30 days beginning on the first billing date until you cancel.  

Pay Annually PLUS Subscription Plan, Insurance Option

By choosing the Pay Annually PLUS Subscription Plan and using insurance to cover your medical visits and medicines, you are agreeing to pay $199 annually starting on the first billing date for the first year of your PLUS Subscription Plan. Your Pay Annually PLUS Subscription Plan will automatically renew annually until you choose to cancel. Your annual recurring charges will be charged every 365 days beginning on the first billing date until you cancel. To the extent permitted by law, the $199 annual PLUS Subscription Plan payment is final and non-refundable.
