What’s Included in My $39 Clinical Assessment Trial Plan?

Please note: *The $39 Clinical Assessment Trial Plan does not include the cost of the Found subscription, any medications, or lab work. Some medications you’re prescribed might be covered by your insurance, but coverage details, pricing, and terms vary by your plan. You may have to pay copays, coinsurance, or a deductible. Prescriptions are at the discretion of your medical provider and medical visits may be asynchronous depending on the state you are located.

Clinical Assessment Trial Plan Terms and Cancellation Policy

Once you begin your text-based consultation, the $39 assessment fee is non-refundable, as allowed by law. After receiving your care plan, you can decide if you want to start with a Found subscription. If you decide to move forward with a subscription, here’s what happens:

• Your monthly subscription starts immediately when you confirm you want to move forward with your care plan and automatically renew each month until you choose to cancel. 
• We will credit the $39 clinical assessment fee toward the first 30 days of your subscription, so your first payment will be $90, due the day that you confirm you want to start your care plan (“First Billing Date”). You authorize Found to automatically charge your credit card for these amounts and all ongoing monthly subscription fees.
• After the first 30 days, you will be charged $129 every 30 days from your First Billing Date until you cancel.

• Custom meal plans, workouts, and daily lifestyle guidance through our mobile app.

How to Cancel

You can cancel your subscription at any time by contacting our customer support team at [email protected]. Your cancellation will be effective within 24 hours. You can also cancel in the app by clicking the "Cancel Membership" link in your Account Details page on the app or the portal. 

All capitalized terms not otherwise defined in these Clinical Assessment Trial Plan Offer Terms and Cancellation Policy “Clinical Assessment Trial Plan Offer Terms” will have the meanings set forth in the Terms. In case of a conflict between the provisions of these Clinical Assessment Plan Offer Terms and the Terms, these Clinical Assessment Trial Plan Offer Terms will control. 

Important: The Clinical Assessment Trial Plan is available only to members who choose to self-pay and is not available to people with in-network insurance, government insurance plans like Medicare, Medicaid, TriCare, MediCal, or similar programs.
