?

Foundayo is a drug approved by the U.S. Food and Drug Administration (FDA) on April 1, 2026, for chronic weight management. It is prescribed for adults with obesity, or who are overweight (a BMI of 27 or higher) with at least one weight-related health condition such as high blood pressure, heart disease, or kidney disease.

Foundayo is a brand name for orforglipron, a small molecule GLP-1 receptor agonist manufactured by Eli Lilly. Unlike Lilly's other weight loss medication, Zepbound, Foundayo is not a peptide — it's a small molecule drug, which means it's absorbed differently by the body and can be taken at any time of day without food or water restrictions.

Foundayo is available as an oral tablet in six doses: 0.8 mg, 2.5 mg, 5.5 mg, 9 mg, 14.5 mg, and 17.2 mg. It is the second GLP-1 pill approved by the FDA for obesity, following the approval of Novo Nordisk's Wegovy pill in December 2025. It was approved in just 50 days — the fastest approval of a new molecular entity since 2002 — under the FDA's Commissioner's National Priority Voucher program.

Foundayo™
work?

When you eat, your gut releases GLP-1, a hormone that helps the pancreas release the right amount of insulin, slows the movement of food through the digestive system, and signals fullness to the brain.

In some people, the body doesn't produce enough GLP-1, or the brain doesn't respond to it as strongly. Foundayo is a synthetic GLP-1 receptor agonist — a drug that activates the same cell receptors as your body's natural GLP-1 hormone, helping trigger the same responses: slowing digestion, reducing hunger, and regulating blood sugar after meals.

What sets Foundayo apart from other GLP-1 medications is its chemical structure. Foundayo is a small molecule, not a peptide. This means it's absorbed directly through the gut without needing special handling — so unlike the Wegovy pill, there are no food, water, or timing restrictions.

Because Foundayo supports a process the body may not be doing well on its own, it may require long-term use.

How to use it

Foundayo is taken once daily. It can be taken at any time of day, with or without food or water restrictions. Swallow tablets whole — do not break, crush, or chew. Follow your care provider's instructions for use.

Dosing

Foundayo is available in six doses: 0.8 mg, 2.5 mg, 5.5 mg, 9 mg, 14.5 mg, and 17.2 mg. Patients typically start at 0.8 mg, increasing to 2.5 mg after at least 30 days, then to 5.5 mg after another 30 days. The dosage may be increased further to 9 mg, 14.5 mg, or 17.2 mg after at least 30 days at each level, based on treatment response and tolerability. Your care provider will determine the right dose and schedule. Please also read the pharmaceutical packaging.

Side effects

Commonly reported side effects of this medication include:

  • Nausea
  • Constipation
  • Diarrhea
  • Vomiting
  • Indigestion
  • Stomach (abdominal) pain
  • Headache

Other potential side effects:

  • Swollen belly (abdominal distension)
  • Fatigue
  • Belching
  • Heartburn
  • Gas
  • Hair loss
  • Decreased appetite
  • Gastroesophageal reflux disease
  • Altered taste

Black Box Warning

Although rare, GLP-1 receptor agonists including Foundayo may cause thyroid C-cell tumors. Foundayo carries a "black box" warning for this risk. If you experience a lump or swelling in the neck, hoarseness, trouble swallowing, or shortness of breath, contact your healthcare provider immediately.

Do not take Foundayo if you have a personal or family history of medullary thyroid cancer or multiple endocrine neoplasia syndrome Type 2 (MEN 2).

Precautions

Rapid weight loss can lead to muscle loss. To protect your muscle mass, it's important to incorporate strength training and regular exercise while taking Foundayo.

If you take oral birth control pills, talk to your healthcare provider before starting Foundayo. Birth control pills may be less effective while taking Foundayo — your provider may recommend an alternative form of contraception for 30 days after starting Foundayo and for 30 days after each dose increase.

If you are pregnant, planning to become pregnant, or breastfeeding, do not take Foundayo. Foundayo may cause fetal harm and should be discontinued when pregnancy is confirmed.

If you are going under general anesthesia or deep sedation, inform your provider that you are taking Foundayo, as it may slow gastric emptying and increase the risk of pulmonary aspiration.

Interactions

Some drugs can interact with Foundayo, including:

  • Insulin and sulfonylureas, which can cause blood sugar levels to drop too low
  • Oral birth control pills
  • Other GLP-1 receptor agonist medications — use of Foundayo with other GLP-1 medications is not recommended

Avoid alcohol while taking this drug, as it can lower blood sugar and may cause dizziness or fainting when combined with Foundayo.
