Do I qualify?
Ages 65 to 75, enrolled in Medicare Part D
BMI of 35+, or a BMI of 27+ with a qualifying health condition (e.g., heart conditions, CKD)
No GLP-1 filled in your Part D plan in 2026, and no condition already treated by a covered GLP-1 (such as type 2 diabetes)
What if I already take a GLP-1?
It depends. The Bridge is built for members starting GLP-1 weight care through Medicare Part D. If you've already filled a GLP-1 in your Part D plan this year, or take one for a condition like type 2 diabetes, you likely won't qualify for the Bridge — but you still have a home at Found. We care for Medicare members outside of Part D too, with competitive self-pay pricing and all the same Found care: board-certified clinicians, ongoing check-ins, and a program tailored to you.
